To help you get the most out of your experience,
registration for our program is required. With multiple locations throughout
central Ohio and weekly programs, we’re certain that you’ll be able to find a
time and place that works for you.
About Registration
A registration form with at least a $75 deposit must
be received before you are considered registered. Once we receive your
registration we will call you to confirm your reservation and answer any
questions you may have.
Payment Information •
Payment in full is preferred at the time of registration; however, you may make
a $75 deposit. You may call the office in advance at any time to pay your
balance. The balance IN FULL must be paid no later than the evening you arrive
at the program. Payment Options:
- Credit or Debit Card
- Money Order
- Cash (Don’t send cash
through the mail. You can drop your cash payment off at our administrative
offices, but please call us for available times)
Payment in full must be
received no later than the Thursday evening you arrive at the Program.
Registering
Online
You can pay by credit card, money order,
or cash. Please fill out
secure online registration
form and it will automatically be sent to our administrative offices. Your
registration form will be “pending” until we receive your full or minimum
payment. Once we receive your payment, a hotel room will be reserved for you.

You also have the option
of downloading the registration form and sending it to us by mail or fax.

Complete the registration form and mail it, along with a money order made out to
Wellness Driver Intervention Program 2758 Sawbury Boulevard Columbus, OH 43235.
or FAX it to (614)791-8726.
A staff member will call you to confirm receipt of your paperwork and to collect your payment/deposit by credit/debit card. If you prefer, you may mail a money order made out to Wellness Driver Intervention Program, under separate cover. Please indicate your preference when our staff member contacts you.
Signed Rules & Regulations
• When you register, you must sign a copy
of Rules & Regulations and send it along with your registration form. If you
register on-line, you must check the box agreeing to the Rules & Regulations.
Cancellation Policy
• If you cancel your scheduled program date after
your deposit/payment has been received and don’t reschedule for another program,
you will be charged a $75 rescheduling fee.
• If you fail to attend your scheduled program date, and don’t notify us 24
hours prior to the program, you will be charged a $75 no-show fee.
• If you reschedule for a future program, you will be refunded $75. We can do
this only one time. |
